Cancellation Policy

En inscrivant mon enfant au programme de basketball de HD Academy, je, sousigne(e), consent a ce qu’il participe a la gamme complete des activités prévues au programme et sorties, et a ce que l’entraineur ou son assistant, dans l’éventualité d’ un accident ou d’une maladie, autorise en mon nom toutes les procedures, y compris le transport et l’admission de mon enfant à l’hôpital et l’administration de traitements qu’il juge essentiels pour la sante et le bien-être de l’enfant sus mentionné. De telles mesures ne doivent être prises que s’il est impossible de rejoindre immediatement le soussigné. J’ai lu, je comprends et j’accepte les conditions de paiement et la politique de remboursement précité. Je me suis renseigné(e) sur la nature des risques et je les accepte, ainsi que l’entière exonération du program HD Academie de toute responsabilité concernant ces risques. * Toutes les photos seront utilisées pour les buts promotionnels. / In registering my child to the HD Academy training program I, undersigned, permit my child to participate in the full range of activities, including outings. I authorize the coach and his assistant, in the event of accident or illness affecting the above-named child, to authorize on my behalf all procedures, including transportation and admission to hospital and necessary treatment therein, as he may deem essential for the care and well being of said child. Such action is taken only when immediate contact with the undersigned cannot be made. I have read, understand and agree to the conditions of payment and policies. I acknowledge that the service contracted may have certain risks. I am aware of any risks, accept these risks and entirely exonerate HD Academy and its partners from any responsibilities regarding these risks.* All photographs will be used for promotional purposes. *
